	Sending this completed form to COMMISSION@dmgardnermusic.com constitutes a final 
	agreement by the individuals or organizations stated above to commission a work from 
	D. M. Gardner.  D. M. Gardner reserves the right to reject this agreement.  Final 
	agreement by D. M. Gardner will sent as a confirmation letter for your records. 
	Should the commission request be cancelled, fees will be based on the progress of the 
	work completed.  Only completed works will be delivered.
